Advance directives are legal documents that allow you to document your wishes for medical care and appoint a trusted person to make decisions for you if you become unable to communicate them yourself. Key types include a Living Will, which specifies your preferred treatments, and a Healthcare Power of Attorney, which designates a healthcare agent to speak for you.

Advance directives vary by state and so it is important to fill out a state-specific advance directive form. It is possible that a living will or durable power of attorney signed in one state may not be recognized in another.
